Tribal Dementia Care Specialist (DCS)

 

Position Summary:

The Tribal Memory Care Coordinator, or Dementia Care Specialist (DCS), works in partnership with the Tribal Health and Human Services agencies and Aging and Disability Resource Center (ADRC) in the Lac du Flambeau Tribe’s service area to ensure that the Indian Health Service (IHS) eligible individuals receive culturally appropriate information and can comfortably and effectively access dementia and long-term care programs and services. The Tribal Memory Care Coordinator will have three primary goals: facilitate dementia-friendly community efforts, train staff in tribal departments to be dementia capable, and support people with dementia and their family members. Daily attendance is an essential function of this position. Regular travel is required to meet the needs of the position.

Principal Duties and Responsibilities:

  • Develop referral relationships with health care providers, dementia diagnostic clinics and long-term care providers. Collaborate actively with local, regional, and statewide dementia organizations.
  • Offer and provide person-centered support when memory screens are positive to assure opportunities for follow-up with a primary care provider in accordance with the individual’s preferences.
  • Provide at least two approved evidence-based or evidence-informed interventions, one specifically designed for family caregivers, as part of ongoing activities. Maintain fidelity to the intervention model and accommodate families’ schedules for meetings (for example, evenings and weekends, when necessary). These will create opportunities for tribal members living with dementia to remain living safely in their own homes and/or community.
  • Serve as a catalyst for developing and implementing strategies to create and sustain dementia-friendly communities in the tribal service area. Provide volunteer training and guidance to expand the reach of the goals of the Dementia Care Specialist (DCS) Program.
  • Provide outreach and awareness to non-healthcare professionals, tribal departments, employers, organizations, and the general community about the tribe, ADRC and available dementia services.
  • Maintain current knowledge of dementia, research findings, new evidence-based interventions, resources and other innovations for people living with dementia and their caregivers.
  • Participate actively in DCS meetings, trainings, and conference workshops.
  • Perform time reporting, collect, and report program data, and contribute to DCS program evaluation.
  • Perform other duties as assigned.

 

This is a full-time salaried opportunity with scheduled hours of Mon-Fri 8:00 AM – 4:30 PM, 40-45 hours per week with a primary work location in Lac Du Flambeau, WI. There will be variations to schedule to accommodate clients and families. This position has a salary range of $48,000 with benefits including Annual/Personal Leave, Holiday Pay, Health, Dental, Life & AD&D, STD and LTD Insurance, FSA and 401(k) Retirement Plan. A standard background check, including a Caregiver Background Check, and drug screen are required before employment. Due to working with health vulnerable elderly people, we prefer candidates who are vaccinated against COVID.

Qualifications: Any equivalent combination of experience and education that would provide the required knowledge and abilities is qualifying. Required: Bachelor of Arts or Science degree, three years’ experience, or a combination of both is required, preferably in the health or human services related field. Will also consider other degrees, certifications in addition to related work experience. At least one year of full-time paid experience working directly with multiple people with dementia and family or informal caregivers (for example, providing direct care, caregiver support, support group facilitation, residential care management, home care) Preferred: Skills and experience in facilitating small group learning and discussion programs. Experience with family caregivers of people with dementia and family dynamics. Experience in developing collaborative relationships, preferably with community organizations and health care professionals. Ability to provide culturally competent services when serving tribal communities. Ability to work independently and as a team member. Ability to adjust work hours to meet with program participants and to provide community and employer outreach and educational programs. Basic computer skills, including the ability to record and report program data and information. Valid Wisconsin Driver’s License and car insurance to travel for community events and to make home visits. Experience working with tribal elders and/or a thorough understanding of elders’ roles in American Indian communities. Experience working with Tribal Governments, Tribal organizations and/or not-for-profit agencies. Ability to work independently and as a team member.
